Student Records

As the school representative for the Green Certificate Program, it is suggested that each of your Green Certificate students be kept on file within your office. Student records may include:

Student Application: As part of the student selection process, the student trainee and parent/guardian express interest in registering
Work Site Approval Form: Assistance for training site evaluation
Parental Consent Forms: Every student needs the consent of his or her parent/guardian to become a trainee
Co-requisite Documentation: Description of the course co-requisite and age requirements
Course Codes: Green Certificate training periods and their corresponding Alberta Education course codes
Green Certificate Registration Form: A copy of the Green Certificate registration form and guidelines on how to properly complete the form
Assessment Forms: Recommended Green Certificate trainee assessment and evaluation guidelines for the school representative
Certification Test Results Forms: An explanation of the regional certification testing process
WCB Claim Forms: An explanation on what to do if an trainee is hurt or injured while training on Green Certificate
School Funding - an explanation of Green Certificate fees and program funding
Refunds Procedure: Procedures to follow if requesting a program fee refund
